Calling Your Insurance

Insurance can sometimes feel stressful… Let’s try to make it easier.

Here’s a list of questions that can help you navigate the phone call to confirm your coverage & benefits:

Make sure to have your insurance card on hand.

1. What's my coverage for outpatient behavioral health services? Does this include telehealth?

2. Do I have a co-pay or co-insurance amount I'm responsible for each session?


3. Do I have a
deductible I have to meet before my sessions are covered?


4. If you’re asked for the following:

  • Provider's name: Doralis Coriano Ortiz, LCPC

  • Provider’s National Provider Identification (NPI) number: 1134675093

  • Therapy practice's name: Fuerza Within, PLLC

5. If you’re asked for the CPT billing codes I'll mainly be using:

  • 90791 for the first session

  • 90837 for the subsequent 55 minute sessions

As a reminder, if we move forward with working together, I will also double check your benefits once I have access to your insurance information.
