Calling Your Insurance
Insurance can sometimes feel stressful… Let’s try to make it easier.
Here’s a list of questions that can help you navigate the phone call to confirm your coverage & benefits:
Make sure to have your insurance card on hand.
You’ll be asked for your Member ID and Group Number.
1. What's my coverage for outpatient behavioral health services? Does this include telehealth?
2. Do I have a co-pay or co-insurance amount I'm responsible for each session?
3. Do I have a deductible I have to meet before my sessions are covered?
4. If you’re asked for the following:
Provider's name: Doralis Coriano Ortiz, LCPC
Provider’s National Provider Identification (NPI) number: 1134675093
Therapy practice's name: Fuerza Within, PLLC
5. If you’re asked for the CPT billing codes I'll mainly be using:
90791 for the first session
90837 for the subsequent 55 minute sessions
As a reminder, if we move forward with working together, I will also double check your benefits once I have access to your insurance information.
